South County Mental Health Center, Inc. seeks an Intake & Evaluation Screener to support diversion, linkage and care coordination for individuals involved with criminal justice and behavioral health systems.

This role helps clients access appropriate levels of mental health care, reduces unnecessary state hospital admissions, and coordinates transitions back into the community. The successful candidate will have forensic mental health experience, strong communication, and the ability to

Qualifikationen

• Bachelor's degree in Human Services or related field required.
• Minimum one year of experience in a mental health setting.
• Working knowledge of Florida Statutes 916 and 394 is preferred.
• Strong written and verbal communication skills required.

Aufgaben

• Diversion & Community Placement Coordination with community resources and transitions.
• Criminal Justice & Court Collaboration including coordination with State Attorney’s and Public Defender’s Offices.
• Intake, Linkage & Transitional Services including gathering charges and treatment history.
• Documentation & Reporting including timely records and monthly reports.
• Other duties as delegated by supervisor.
