Job summary

The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) seeks a Community Clinician to deliver clinical and case management services for adults with complex behavioral health and substance use needs in the Stamford area.

You will work on a multidisciplinary team, providing trauma-informed, person-centered, recovery-focused support, and contribute to treatment goals with direct client contact and community-based services.

Qualifications

• Bachelor’s degree in health care or related field with experience in behavioral health.
• Substitutions may apply for advanced degrees or licensed professionals.
• Experience with case management and counseling in community settings.

Responsibilities

• Implement individualized treatment plans for clients on caseload.
• Provide case management and connect clients with community resources.
• Conduct intake assessments and participate in counseling sessions.
• Monitor progress, medication compliance, and coordinate with clinicians.
• Offer crisis intervention and referrals as needed.
• Educate clients in daily living skills to promote independence.
