Job summary

Aurora Mental Health & Recovery (AMHR) is seeking a Case Manager for the ForFACT Team to deliver outreach, care coordination, advocacy, and individualized support in the community. This role emphasizes an ACT-informed, person-centered approach and collaboration with a licensed therapist and fellow case managers to support clients with complex needs.

You will meet clients where they are, build therapeutic relationships, and contribute to reducing recidivism and improving engagement and outcomes

Qualifications

• Bachelor’s degree in psychology, social work or related human services field.
• OR Bachelor’s degree in unrelated field with at least one year of behavioral health experience.
• Demonstrated interest in behavioral health field.
• Knowledge of community resources.
• Effective organizational and written/verbal communication skills.

Responsibilities

• Serve as payee for clients who require assistance to manage their finances.
• Help clients meet basic needs and assist with income, medical, housing benefits applications.
• Arrange transportation to ensure access to services.
• Provide psychosocial services to improve clients’ functioning.
• Assess strengths/symptoms and help set goals and plans.
• Instruct daily living skills such as medication use, diet, hygiene, budgeting, and transportation.
• Develop social skills and leisure activities for clients.
• Assist in finding employment and vocational services as appropriate.
• Support families and community partners to maximize benefits.
• Leverage natural supports like networks and churches to engage treatment.
• Educate clients about available community services.
