Job summary

Special Health Resources is seeking a Primary Care Provider (FNP or PA) to join Tyler Care Clinic on Troup Hwy & Midtown, Tyler, TX. This full-time, exempt role is onsite with a focus on compassionate, community-based outpatient care and building long-term patient relationships.

You will deliver the full scope of primary care services to adults, including diagnosis, treatment, preventive care and coordination of care within a dedicated clinical team.

Qualifications

• Certified Family Nurse Practitioner (FNP or PA) with current license.

Responsibilities

• Provide the full scope of primary care services for adult patients, including diagnosis, treatment and health maintenance.
• Coordinate care with other clinic teams and services to support comprehensive outpatient care.
• Emphasize preventive care, patient education and health maintenance during visits.

• Incentive Bonus Program: Eligibility to participate in the organization incentive bonus program, subject to the terms and conditions specified in the incentive bonus plan document.
