Job summary

HHM Health in Dallas/South Dallas is hiring a Family Nurse Practitioner to deliver comprehensive primary care across the lifespan. You will assess, diagnose, treat, and prevent disease, collaborating with physicians and a diverse care team to provide patient-centered care.

Qualifications

  • Current advanced practice license to practice in Texas.
  • National board certification as Family Nurse Practitioner (FNP-BC or FNP-C) is a plus.
  • Experience in primary care, family medicine, community health, or outpatient settings preferred.
  • Experience serving multicultural and multilingual patient populations.

Responsibilities

  • Conduct comprehensive patient assessments including histories and physical exams.
  • Diagnose and treat acute and chronic illnesses across all ages.
  • Order, perform, and interpret diagnostic tests and lab results.
  • Develop and implement individualized treatment and care plans.
  • Prescribe medications per state regulations and scope of practice.
  • Provide preventive healthcare services including immunizations and screenings.
  • Collaborate with physicians and interdisciplinary teams to ensure quality care.

We are seeking a Family Nurse Practitioner with a passion for service excellence to join our Family Medicine Healthcare team in the South Dallas area. The Family Nurse Practitioner (FNP) provides comprehensive primary healthcare services to patients across the lifespan, including assessment, diagnosis, treatment, and preventive care. The FNP collaborates with physicians, nurses, and interdisciplinary healthcare teams to deliver high-quality, patient-centered care while promoting wellness and disease prevention.

  • Conduct comprehensive patient assessments, including medical histories and physical examinations.
  • Diagnose and treat acute and chronic illnesses across all age groups.
  • Order, perform, and interpret diagnostic tests and laboratory results.
  • Develop and implement individualized treatment and care plans.
  • Prescribe medications and therapiesin accordance withstate regulations and scope of practice.
  • Provide preventive healthcare services, including immunizations, screenings, and health education.
  • Manage chronic conditions such as diabetes, hypertension, asthma, and obesity.
  • Counsel patients and families on disease prevention, nutrition, lifestyle modifications, and wellness strategies.
  • Collaborate with healthcare providers and staff to ensure continuity and quality of care.
What you need to succeed
  • BSN/MSN from an accredited school of nursing or equivalent; current advanced practice license to practice in the state of Texas
  • Registered and have current DEA, DPS and other such certificates to legallyoperateas apractitioner in the State of Texas or be able to obtain
  • National board certification as a Family Nurse Practitioner (FNP-BC or FNP-C) is a plus
  • Current and unrestricted Nurse Practitioner licensure in the state of Texas
  • Experience in primary care, family medicine, community health, or outpatient settings preferred.
  • Excellent patient engagement and communication skills
  • Experience in community health centers and/or non-profits is preferred
  • Experience serving multicultural and multilingual patient populations
  • Experience with eClinicalWorks or similar
  • MS Office Suite skills (Outlook, Teams, Word,PowerPointand Excel)
  • Have a servant heart with the desire to make a positive impact
