Job summary

Avenue at Wooster, a post-hospital rehabilitation and skilled nursing facility, is hiring Floor Nurses (LPNs or RNs) who thrive in a team environment and are committed to quality care. You will provide direct care based on individualized plans and support residents daily.

Qualifications

• Current LPN or RN license in the state of employment.
• CPR certification (AHA, Red Cross, or equivalent).
• One year of long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to lift up to 50 lbs and work flexible hours.

Responsibilities

• Deliver nursing care based on individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor and document resident conditions and responses.
• Assist with admissions, discharges, and transfers.
• Supervise nursing assistants and collaborate with care teams.
• Participate in care planning and assessments.
• Respond to emergencies and ensure regulatory compliance.
