Hickory Treatment Centers in Indianapolis, United States, is seeking a Floor Nurse (LPN/RN) to cover the 11:00pm – 7:30am shift, caring for adults with addictions and behavioral health issues. Under the Director of Nursing, you will perform comprehensive physical health assessments on intakes and formulate an integrated plan of care including physician orders, medications, education and treatments.

Responsibilities

• Performs Nursing Assessments on admission.
• Performs intake assessments and ongoing client evaluations.
• Evaluates clients to ensure medical needs are met.
• Administers medications and ensures safety.
• Documents medication administration with accuracy.
• Performs CIWA and COWS assessments.
• Performs pain assessment and manages pain.
• Maintains infection control and safety practices.
• Documents patient education and follows discharge procedures.
• Communicates using Read Back for verbal orders.

Job Type: Full - Time

Shift Schedule: 11:00pm - 7:30am

About the Role:

The Licensed Practical Nurse (LPN)/ Registered Nurse (RN) will be responsible for the healthcare of female and/or male clients suffering from addictions and behavioral health issues.

Under the immediate supervision of the Director of Nursing, the LPN will perform comprehensive physical health assessments on all intakes and formulate a plan of care that includes physician orders, medications, education, and treatments.

Job Qualifications:

• Valid LPN or RN completed from an accredited school of Nursing.
• Prior experience as an LPN or RN preferably with hospital and behavioral health experience
• Certified BLS provider obtained within 1 month prior to hire.
• Ability to maintain strict client confidentiality and HIPAA compliance.

Key Responsibilities:

Clinical Assessments & Monitoring

• Performs Nursing Assessment on admission
• Performs intake assessments
• Evaluates clients on an ongoing basis to ensure medical needs are met
• Performs CIWA and COWS appropriately
• Performs pain assessment and is knowledgeable about pain management
• Performs point-of-care urinalysis and pregnancy screens
• Performs glucose monitoring
• Administers two-step Tuberculin PPD test
• Performs COVID-19 testing

Medication Management & Safety

• Understands administration of high-risk medications
• Uses two identifiers when administering medications
• Documents medication administration
• Receives medications from pharmacy and stores appropriately
• Conducts narcotics count and maintain emergency drug kit
• Reports medication errors or “near misses’ to Director of Nursing

Documentation & Communication

• Utilizes “Read Back” when taking verbal orders
• Documents client education
• Assessing, responding, and documenting suicide risks
• Follows admission and discharge procedures

Infection Control & Safety Practices

• Performs appropriate hand hygiene throughout the shift
• Uses personal protective equipment (PPE) appropriately
• Follows workplace injury procedures
• Reports safety hazards to supervisor and/or Executive Director

Compliance, Professional Standards & Quality Improvement

• Maintains a current nursing license (LPN/RN)
• Participates in improvement activities

Compensation:

$29.00 - $33.00 Hourly
