Job summary

Hickory Recovery Network is seeking a Floor Nurse (LPN/RN) to provide care for adults with addictions and behavioral health issues in Indianapolis. The role involves comprehensive assessments, care planning, and collaboration with the Director of Nursing.

Responsibilities include medication management, documentation, and ensuring safety and infection control. A current LPN/RN license and BLS are required, with experience in hospital/behavioral health settings preferred.

Qualifications

• Valid LPN or RN license from an accredited nursing program.
• Experience as LPN/RN, preferably in hospital and behavioral health settings.
• BLS certification obtained within 1 month prior to hire.

Responsibilities

• Perform nursing assessments on admission.
• Conduct intake assessments and ongoing medical evaluations.
• Monitor clients to address medical needs; follow physician orders and care plans.
• Administer and monitor medications, including high-risk meds.
• Perform CIWA, COWS, pain assessments, and manage pain.
• Conduct point-of-care tests (urinalysis, pregnancy screens) and glucose checks.
• Administer TB testing and COVID-19 testing.
• Document medication administration and maintain narcotics counts.
• Ensure Read-Back for verbal orders and follow admission/discharge procedures.
• Maintain hand hygiene and report safety hazards.
• Keep nursing license current and adhere to professional standards.

Shift Schedule:11:00pm - 7:30am

About the Role:

The Licensed Practical Nurse (LPN)/ Registered Nurse (RN) will be responsible for the healthcare of female and/or male clients suffering from addictions and behavioral health issues.

Under the immediate supervision of the Director of Nursing, the LPN will perform comprehensive physical health assessments on all intakes and formulate a plan of care that includes physician orders, medications, education, and treatments.

Job Qualifications:

• Valid LPN or RN completed from an accredited school of Nursing.
• Prior experience as an LPN or RN preferably with hospital and behavioral health experience
• Certified BLS provider obtained within 1 month prior to hire.
• Ability to maintain strict client confidentiality and HIPAA compliance.

Key Responsibilities:

Clinical Assessments & Monitoring

• Performs Nursing Assessment on admission
• Performs intake assessments
• Evaluates clients on an ongoing basis to ensure medical needs are met
• Performs CIWA and COWS appropriately
• Performs pain assessment and is knowledgeable about pain management
• Performs point-of-care urinalysis and pregnancy screens
• Performs glucose monitoring
• Administers two-step Tuberculin PPD test
• Performs COVID-19 testing

Medication Management & Safety

• Understands administration of high-risk medications
• Uses two identifiers when administering medications
• Documents medication administration
• Receives medications from pharmacy and stores appropriately
• Conducts narcotics count and maintain emergency drug kit
• Reports medication errors or “near misses’ to Director of Nursing

Documentation & Communication

• Utilizes “Read Back” when taking verbal orders
• Assessing, responding, and documenting suicide risks
• Follows admission and discharge procedures

Infection Control & Safety Practices

• Performs appropriate hand hygiene throughout the shift
• Follows workplace injury procedures
• Reports safety hazards to supervisor and/or Executive Director

Compliance, Professional Standards & Quality Improvement

• Maintains a current nursing license (LPN/RN)
