Job summary

CAPS & Maple City is seeking a Floor Nurse to join a cross-disciplinary team delivering patient-centered care in Goshen, Indiana. The role involves direct nursing care under supervision, collaboration with clinicians, and engaging patients through various channels to promote healthier lifestyles.

You will complete documentation accurately, coordinate with external care partners, and participate in ongoing trainings.

Qualifications

• Must hold a nursing degree and be licensed in Indiana.
• Provide direct, basic nursing care under RN or physician supervision.
• BLS certification is required; Trauma Informed Care and Motivational Interviewing training preferred.

Responsibilities

• Collaborates with a cross-disciplinary care team to address patients' holistic needs.
• Engages patients via phone, MyChart, or in-person visits using Motivational Interviewing and Trauma Informed Care approaches.
• Completes timely and clear documentation.
• Refills patient medications and prepares patient forms/letters with providers.
• Coordinates with outside entities for patient care.
• Maintains competency by participating in scheduled trainings.
• Performs other duties that support the team as assigned.

Job Summary

The Floor Nurse is a member of a cross-disciplinary team that includes health care providers, medical assistants, nurses, behavioral health providers and other support staff. They support the mission and values of CAPS & Maple City by collaborating with team members across disciplines to facilitate appropriate quality care for patients while also empowering and encouraging self-motivation for a healthier lifestyle.

Duties and Responsibilities

• Collaborates with the cross-disciplinary care team to address the wholistic needs of patients


• Engages patients to address patient needs via phone/MyChart/in-person, utilizing Motivational Interviewing and Trauma Informed Care approaches


• Completes documentation in a timely and clear manner


• Refills patient medications


• Completes patient forms and letters in collaboration with provider


• Coordinates with outside entities for patient care


• Maintains skills competency by participating in scheduled trainings


• Other duties that support our shared work, as assigned

Requirements

Required Skills and Abilities

• Demonstrates effective interpersonal skills

Education and Experience

• Nursing degree


• Provide direct, basic nursing care under RN or physician supervision


• State of Indiana licensure


• Basic Life Support Certification required


• Trauma Informed Care approach preferred; training supported


• Motivational Interviewing training preferred; training supported
