Job summary

Legacy Community Health seeks a Health Navigator - Float to travel among Legacy clinics, supporting patients with access to care and HIV prevention services. You will educate patients, coordinate referrals, and work with the care team to improve outcomes across communities.

The role emphasizes patient-centered care, collaboration, and community engagement, requiring flexible travel and strong communication in a diverse setting.

Qualifications

• High school diploma or equivalent required.
• 3–5 years of relevant experience.
• Bachelor's degree in a related field with 1+ year preferred.
• Experience performing point-of-care testing in clinical/community settings.
• Demonstrated HIV care and prevention knowledge.
• Experience educating diverse clients in group or one-on-one settings.
• Proficient with Electronic Health Records Systems.

Responsibilities

• Develop and implement health education programming for individuals and groups.
• Use motivational interviewing to support health goals.
• Provide information, referrals, and assistance to improve access to services.
• Perform point-of-care testing for HIV and other conditions.
• Collaborate with the clinic care team to support patient needs.
• Assess behavioral, social, and medical factors affecting patients.
• Create and manage referral processes and resource lists.
• Promote patient-centered care within a primary care model.
• Document all services in EHRs and manage schedules efficiently.
• Meet grant- and contract-related performance metrics.
• Travel among Legacy clinics to support community needs.
• Take on additional duties to support Legacy’s mission.
