Job summary

Qualifications

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate complex patient conditions and document treatment responses.
• Monitor vital signs and record abnormalities.
• Perform routine assessments and ensure patient well-being.
• Facilitate patient access to resources and disease management support.
• Conduct medication reviews for side effects and interactions.
• Document patient history and care in EMR with accuracy.
• Communicate effectively with patients, families, and healthcare teams.
