Qualifications

• NH RN licensure is required.
• Current BLS/CPR training.
• Prior work experience in healthcare setting is required.

Responsibilities

• Carry out nursing process in person, by telephone, and via telehealth modalities.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress.
• Collaborate to design and implement a plan of care with the interdisciplinary team.
• Administer prescribed medications, treatments, vaccines, and perform related tests.
• Educates patient on self-management activities based on the care plan.
