One Community Health seeks a Registered Nurse to provide direct patient care across our Midtown, Carmichael, and Arden sites. This per diem float role supports a diverse patient population and requires flexibility to work across locations, including Saturdays.

Responsibilities include nursing assessment, documentation in the EMR, triage, patient education, medication administration, and coordination of follow-ups. California RN license and CPR/BLS are essential.

Qualifikationen

• Assess, plan and deliver nursing care with the care team.
• Document all patient interactions in the EMR accurately.
• Provide in-person and telephone triage using approved protocols.
• Deliver culturally competent care to a diverse population.
• Administer medications as ordered and manage related tasks.
• Coordinate post-hospital follow-up and hypertension care.
• Support continuity of care across multiple clinic sites.

• Apply the nursing process to assess and intervene in patient care.
• Document interactions and plan in the electronic medical record.
• Triaging patients by phone or in person per protocols.
• Support diverse patients with chronic disease management and education.
• Coordinate follow-ups and specialty appointments as needed.
• Assist with venipuncture, wound care, and other procedures.
