One Community Health is seeking a Per Diem Registered Nurse to provide direct patient care across Midtown, Carmichael, and Arden clinics. The role operates as a float with a minimum of 6 shifts per month and requires availability on Saturdays.

You will apply the nursing process, document interactions in the EMR, and participate in triage and patient education while supporting a team-based model of care across diverse populations.

Qualifications

• Associates degree in nursing or higher required.
• California RN license active and in good standing.
• Current CPR/BLS certification required.
• Minimum 1 year of clinical nursing experience.

Responsibilities

• Provide direct patient care under Nurse Manager supervision using the nursing process.
• Document all patient interactions in the EMR with accuracy and timeliness.
• Perform triage by phone and in person following approved protocols.
• Deliver culturally competent care across newborn to geriatric populations.
• Coordinate follow-up appointments and referrals as needed.
• Administer medications as ordered; manage venipuncture and wound care.
• Support team-based care with MAs and LVNs and ensure continuity of care.
• Schedule follow-ups and respond to patient communications per protocol.

Per Diem Registered Nurse

Under the direct supervision of the Nurse Manager, the Registered Nurse is responsible for providing direct patient care to a diverse patient population across the continuum of care.

Location:

This float position provides assistance across all One Community Health clinic sites, including Midtown (95811), Carmichael (95608), and Arden (95825).

Schedule:

• Per Diem
• Must be available to work a minimum of 6 shifts per month
• At least one (1) shift per month must be on a Saturday

Note:

This is a per diem float position and does not guarantee a set schedule or hours. Availability across multiple clinic locations is required. Not benefits eligible.

Essential Functions:

• Application of the Nursing process to assess, plan and carry out interventions in the management of patient’s care in collaboration with the multidisciplinary care team including medical providers, mental health/behavioral health providers, case management, dental, optometry, registered dieticians, clinical pharmacists, health educators etc.
• Detailed, systematic documentation of all patient interactions in the electronic medical record.
• In-person and telephone triage and advice supported by recognized industry protocols; approved by the Chief Medical Officer and compliant with clinic policy.
• Nursing support for diverse population ranging from newborn to geriatric using culturally competent care.
• Coordination of post-hospital follow-up appointments for primary care patients.
• Nurse-led hypertension management through comprehensive One Community Health program.
• Results review, including lab and imaging results for issues ranging from chronic diseases to sexual health.
• Patient education and counseling for chronic disease management, medication adherence, preventive care, and wellness promotion.
• Medication administration as ordered by qualified providers; including PO, inhaled, IM, IV, and intradermal medications.
• Procedures and patient care as ordered, including venipuncture, wound care, port flushes, etc.
• Active support of health team members for the continuity of care provided to all patients at One Community Health.
• Patient scheduling of follow-ups and specialty appointments.
• Coordinating and follow-up with patient calls and EHR messaging within timelines set by clinic protocols.
• Leadership to clinical support staff including MAs and LVNs in a team-based model of care.

Minimum Requirements

• Associates in Nursing
• 1 year clinical experience
• Current active license to practice as an RN in the state of California.
• Active CPR or BLS Certification

Preferred

• Experience in Infectious Disease care
• Experience in gender affirming care
• Bachelor’s degree in nursing
• 1 year acute care experience
• Experience with Epic EMR
