Job summary

Circle of Life is seeking a Float Registered Nurse Case Manager to join our Waite Park team. You will provide clinical coverage, travel to clients’ homes, and collaborate with an interdisciplinary team to ensure high-quality care.

This role supports our mission to help clients stay independent in their homes and communities. The position requires a MN RN license, willingness to travel in the Twin Cities and surrounding area, and a passion for direct patient care within a supportive care team

Responsibilities

• Float nurse coverage for Waite Park/Central Region and Twin Cities/Metro offices.

Position Duties Include

• Serve as a float nurse supporting our Waite Park/Central Region and Twin Cities/Metro offices by providing clinical coverage for staffing needs, onboarding support, and organizational growth as needed.
• Manage and schedule your visits. You’ll travel around the area to provide 1:1 nursing services to clients in their home, traveling is required.
• Collaborate with an interdisciplinary teamto help provide the best care possible to our clients.
• Provide clinical supervision to our caregivers, making sure they are following the care plan and inspiring them to provide excellent care.

Qualifications

Qualified applicants need a validMNRegistered Nurse license and interest in visiting clients in their homes in the Twin Cities and surrounding area (traveling is required for this role). This position is well suited to an individual that enjoys direct patient contact, supporting a dedicated care team doing good in our communities.
