Qualifications

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and medication administration.
• Provide education for patients and caregivers on chronic conditions and preventative care.
• Conduct medication reviews during nurse-led visits to ensure safety and adherence.
• Conduct onsite testing and use data to identify gaps in preventative screenings.
• Review high-priority abnormal lab results and act promptly.
• Partner with providers and interdisciplinary team for complex care plans.
• Use standardized protocols for medication management and renewals.
• Delegate tasks to medical assistants and phlebotomists as needed.

About This Role

What You'll Do

• Support patient assessment and prioritization for walk-in and telephonic triage.

Requirements

• Education: Active, Unrestricted Registered Nurse Licensure in the state that you reside
• Experience: 2+ years of healthcare experience, working as an RN preferred
• Licenses & Certifications: BLS or CPR Certification required
• Licenses & Certifications: Electronic Medical Record (EMR) experience required
