Job summary

Qualifications

• Effective English communication skills, both written and oral.

Responsibilities

• Coordinates an interdisciplinary plan of care.
• Ensures 100% assigned patients have a documented plan of care.
• Delegates aspects of care to team members within scope of practice.
• Establishes a therapeutic relationship with patient/family.
• Participates in orienting and mentoring associates.
