Qualifications

• Active, unrestricted US nursing license in Illinois.
• Current CPR certification.
• Ability to communicate clearly with residents, families and colleagues.
• Willingness to adapt to multiple units and care settings.

Responsibilities

• Deliver direct patient care per care plans and physician orders.
• Supervise and support CNAs to ensure high-quality care.
• Administer medications and treatments as prescribed.
• Monitor residents and report changes promptly.
• Coordinate admissions, discharges and transfers.
• Maintain accurate documentation in PCC.
• Foster resident-centered care and family relationships.
