What You Will Do

• Practice independently within the community as part of an interdisciplinary care team
• Conduct comprehensive patient assessments to determine needs and develop treatment plans and outcomes
• Deliver personalized rehabilitation therapy in patients' homes or care facilities
• Continuously evaluate patient needs and adjust plans of care accordingly

Qualifications

Minimum of one-year physical therapy experience or successful completion of probation as a VNS Health Rehab Intern

License to practice as a Physical Therapist in New York State

Valid driver's license required for areas outside Manhattan

Pay Range per Visit

For positions outside the Bronx, the range is $70-$75 per visit. For Bronx-based positions, the range is $90-$95 per visit.

Salary Range

USD $85,000 - $127,500 per year
