Job summary

Optima Care Fountains in Secaucus, NJ seeks an experienced LPN to support residents during the 3:00 pm – 11:00 pm shift. This floater role covers needs across the unit and requires flexible scheduling based on patient care demands.

The successful candidate will provide nursing care, administer medications, assist physicians, and collaborate with the care team to ensure comfort and safety for residents. A valid RN/LPN license is required.

Qualifications

• Must be licensed as RN or LPN in New Jersey.
• Experience providing direct patient care and monitoring.

Responsibilities

Optima Care Fountains is seeking a dedicated and experienced LPN to help care for residents in their Secaucus location.

This position will be a floater position to cover on the 3:00 pm - 11:00 pm shift. Schedule will be determined by need.

Duties and Responsibilities

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Physicians and others with diagnostic procedures and treatments.
• Reinforce supports and provides health care instructions to patients and their families.
• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.
• Assists in the admission, transfer, and discharge of patients.
• Observes and reports patients' vital signs, symptoms and general condition to the appropriate individual(s).
• Records on patient charts observation of patient’s responses to nursing care rendered.
• Maintains environmental conditions conducive to the comfort and safety of patients.
• Assists with unit management and/or assume charge of unit as assigned.
