Job summary

Concierge Home Care in Jacksonville, FL is seeking a licensed Speech-Language Pathologist (SLP) to deliver in-home therapy focused on communication, cognition and swallowing. This role offers flexible weekday scheduling, autonomy over your daily plan, and a pay-per-visit model designed to reward productivity.

You will conduct assessments, develop and implement treatment plans, educate patients and families, and collaborate with physicians and the care team.

Qualifications

• Active Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.

Responsibilities

• Conduct comprehensive speech, language, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on communication, cognition, and swallowing disorders.
• Educate patients and caregivers on techniques to improve functional outcomes.
• Collaborate with physicians and interdisciplinary care teams.
• Complete Medicare OASIS documentation as required.
• Document visits efficiently using AI-assisted tools.
