Job summary

UnitedHealth Group in Massachusetts is seeking a Physical Therapist for Home Health to assess and manage patient care needs related to mobility, endurance, and pain. You will help develop and implement a treatment plan, provide interventions, and monitor progress in a home health setting.

The role requires current PT licensure, valid driver’s license with reliable transportation, and ability to obtain CPR certification if not current.

Qualifications

• Current Physical Therapy licensure in state of practice
• Current driver’s license and vehicle insurance or access to dependable transportation
• Preferred: CPR certification or ability to obtain within 90 days of hire

Responsibilities

• Evaluates patient functional status and PT needs; assists in development of therapy plan of care
• Provides PT services within scope of practice per state laws and plan of care
• Observes and reports patient response to treatment to supervisor/physician
• Treats patients to relieve pain, restore function, and maintain max functioning
• Instructs patient and family about plan of care, limitations, and progress
• Visits patients per Plan of Treatment and documents progress notes
• Completes evaluations and develops PT plan of care per state guidelines
• Reports outcomes, goals, and projected frequency of care
