Job summary

Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is hiring a Speech Language Pathologist. The role involves performing comprehensive speech therapy evaluations, developing treatment plans with goals and modalities, and coordinating with MD orders for treatments, discharges and recertifications.

You will document progress, participate in interdisciplinary meetings, supervise students as needed, and ensure compliance with HIPAA and facility policies.

Qualifications

• Graduate of an accredited university with an MA or MS in communication disorders.
• Holds current state license/registration where applicable.
• Successfully completed CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on standardized evaluation formats accurately.
• Develop comprehensive treatment plans with goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment using approved modalities and maintain safety.
• Document progress in approved formats in a timely manner.
• Update and document monthly treatment plans.
• Document discharge and in-service notes as indicated.
• Document caregiver or discharge in-services as needed.
• Complete documentation required by the facility.
• Prepare care plans and information for MDS as needed.
• Attend facility and rehab meetings as directed.
• Participate in facility committee meetings as indicated.
• Act as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and perform screenings as indicated.
• Bill patients ethically and accurately for services rendered.
• Cover other therapists’ caseload during absences as necessary.
• Supervise junior staff or students per state and federal guidelines.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education to rehab staff and facility staff as needed.
• Comply with facility policies, procedures and safety guidelines.
• Maintain patient confidentiality and HIPAA compliance.
• Report to work on time and maintain a professional image.
• Adhere to productivity requirements and submit reports timely.
• Comply with HR policies including lunch, overtime, and time-off notices.
• Maintain a positive work atmosphere and foster good relations.
• Perform other duties assigned by supervisor or Regional Manager.
• Adhere to all facility procedures and systems.

MA/MS in communication disorders (ASHA/state board)
