Job summary

Yanceyville Rehabilitation And Healthcare Center is actively seeking a Speech Therapist to join our rehab team in Yanceyville, NC. CFY candidates are encouraged to apply and will work under MD orders to evaluate and treat residents.

The therapist will document evaluations, treatment plans, progress notes and discharges while collaborating with an interdisciplinary team to meet care goals.

Qualifications

• Graduate of an accredited university with an MA, or MS in communication disorders recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying treatment as indicated.
• Document patient progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient discharge and in-service as indicated.
