Job summary

BrightStar Care of West Bend is seeking a Registered Nurse to provide compassionate home health care in the West Bend area. You will assess, plan, and deliver nursing care to clients in their homes, coordinating with a Director of Nursing and healthcare team.

Eligibility requires Wisconsin RN licensure, strong clinical and communication skills, and the ability to work weekends or holidays as needed. We offer flexible schedules, weekly pay, and opportunities for advancement.

Qualifications

• Wisconsin RN license required.
• Experience in home health or nursing care preferred.
• Excellent interpersonal and communication abilities.

Responsibilities

• Provide nursing care within the scope of the Nurse Practice Act.
• Conduct initial evaluation visits and regular re-evaluations of patient needs.
• Develop and revise plans of care in coordination with the Director of Nursing.
• Deliver nursing services and preventive/rehabilitative procedures.
• Document care with progress notes and summaries.
• Inform Director of Nursing and physicians of changes in patient condition.
• Counsel clients and families on nursing needs and care plans.
• Supervise and educate other nursing staff as directed.
