Job summary

BrightStar Care in Wisconsin is seeking a Registered Nurse or Licensed Practical Nurse to provide in-home nursing services across West Bend and surrounding areas. You will perform skilled nursing assessments, develop and revise plans of care, and coordinate with the Director of Nursing to ensure high-quality patient outcomes.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered licensure to practice professional nursing in Wisconsin.
• Excellent interpersonal skills with patients and families.
• Strong clinical judgment and ability to work independently.
• Compliance with HIPAA and professional standards.

Responsibilities

• Provides care as defined within the scope of the state’s Nurse Practice Act using sound judgment and critical thinking.
• Initiates and reevaluates the plan of care in collaboration with the Director of Nursing.
• Delivers skilled nursing services and rehabilitative procedures as needed.
• Keeps clear clinical notes and communicates changes in patient condition.
• Assists in staff training and ensures quality, continuity, and safety of care.
