Job summary

BrightStar Care West Bend is seeking a part-time Nurse (LPN/RN) to provide in-home nursing care in West Bend, WI. This position offers a flexible schedule with evenings and occasional weekend hours.

You will assess and monitor clients, initiate and revise plans of care, and work under supervision of the Director of Nursing to ensure quality, safety and regulatory compliance. A Wisconsin RN or LPN license is required, along with solid communication skills and a commitment to compassionate,

• Provides care as defined within the scope of the state’s Nurse Practice Act using sound judgment and critical thinking.
• Initiates the plan of care and makes necessary revisions, keeping the Director of Nursing updated on the revisions.
• Assists in the development of organization goals and administers agency policies and procedures.
