Job summary

Grand River Health in Rifle, CO is hiring a PRN Physical Therapy Assistant to support the Rehabilitative Services team. You will deliver prescribed treatments under a licensed Physical Therapist, helping reduce pain, improve mobility and restore function.

Required: Colorado PTA license (DORA) and AHA BLS certification; at least 1 year in rehab settings preferred. You will use Meditech, QHN, Cedaron and other EHR tools to document care and coordinate with the team.

Qualifications

Current Colorado PTA DORA license required.
Current American Heart Association BLS certification required upon hire and must remain current.
At least 1 year of experience in a Rehabilitative Services setting preferred.

Responsibilities

Reviews patient charts and prepares patients, residents, and equipment for scheduled therapy services.
Implements treatment plans as directed by the supervising Physical Therapist.
Monitors patient responses and communicates changes to the supervising therapist.
Documents treatments and changes in condition in accordance with department standards.
Collaborates with Therapists, Attendants, Registrars, and team members and follows up when patients miss appointments.
Performs other duties as assigned.
