Well Care Health in Socastee, SC seeks a registered nurse for home health care. You will assess patients, develop and evaluate plans of care, and coordinate with a multidisciplinary team to ensure quality outcomes.

Demonstrates strong communication, organizes work, and maintains flexibility to meet scheduling needs including weekends and holidays. Requires valid NC/SC RN license and CPR, with at least 1 year RN experience and 2+ years total clinical experience.

Formación

• Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Current license to practice in SC/NC; CPR certification.
• 1 year RN experience; total 2+ years clinical experience.
• Home health experience preferred.
• Valid NC driver's license and vehicle required.
• Ability to communicate in English; strong interpersonal skills.
• Ability to lift and move patients; physical stamina required.

Responsabilidades

• Assess, interpret, plan, implement and evaluate patients by age and diagnosis.
• Coordinate care with a multidisciplinary team to manage caseload.
