Job summary

Team Select Home Care of Indiana, LLC seeks an attentive Home Health Aide to provide skilled personal care and ADLs in patients' homes under RN supervision. You will report to the RN and contribute to coordinated care plans while maintaining infection control and timely documentation.

Requires a high school diploma or GED, 18+ years old, CPR and state-required certifications. Benefits include health insurance options, 401(k), paid training, and weekly pay in a supportive home-care team.

Qualifications

• High School Diploma or GED required.
• CPR certification and health certifications per state requirements.
• Ability to follow directions and communicate with patients respectfully.

Responsibilities

• Provide direct patient personal care and ADLs as allowed by plan of care.
• Assist with household chores, meals, and daily tasks as assigned.
• Assist with simple procedures as delegated and with ambulation/exercises.
• Support self-administration of medications as permitted by state rules.
• Maintain infection control practices and report changes to the RN.
• Complete visit records timely and participate in staff meetings.
• Follow agency policies, protect confidentiality, and document care accurately.
