Job summary

LilyCare Hospice of New Hampshire is seeking a Hospice Social Worker (MSW) to join our interdisciplinary team in Bedford, NH. This field-based role visits patients at homes and facilities, providing psychosocial support to patients and families.

The ideal candidate holds an MSW from a CSWE-accredited program, a NH social work license (LICSW/LCSW preferred), and at least two years of geriatric experience, with hospice/palliative care preferred.

Qualifications

• Master's degree in Social Work (MSW) from a CSWE-accredited program preferred.
• BSW from an accredited program considered with MSW supervision.

Responsibilities

• Complete psychosocial assessments of patients and families and contribute to the Plan of Care.
• Provide counseling and emotional support to patients, families, and caregivers during grief, conflict, or crisis.
• Evaluate insurance coverage and financial needs and connect families with resources and assistance programs.
• Participate in IDT meetings and keep the team updated on psychosocial changes and care plan adjustments.
• Make referrals to community agencies and coordinate services as needed.
• Support bereavement follow-up services for families after a patient's death.
• Maintain accurate documentation of visits and services.
• Assist with training and orientation of volunteers and staff, and support programs as needed.
• Participate in quality improvement activities.
