Job summary

Ramseur Rehabilitation and Healthcare Center in Ramseur, NC is seeking a Part-time Physical Therapist to join our rehabilitation team. You will assess residents, create individualized treatment plans, and provide ongoing therapy using a range of modalities while documenting progress and outcomes.

The role involves coordinating with the care team, ensuring compliance with all facility policies, and delivering high-quality patient care in a long-term care setting.

Qualifications

• Graduate with a PT degree recognized by the APTA.
• Holds current license and/or registration in the state.

Responsibilities

• Provide comprehensive physical therapy evaluations based on MD orders.
• Develop and implement treatment plans with goals, frequency, duration and modalities.
• Document evaluations, progress notes and discharge summaries accurately and timely.
• Coordinate with the interdisciplinary team and participate in meetings as needed.
• Maintain patient confidentiality and comply with facility policies.
