Job summary

Ramseur Rehabilitation and Healthcare Center in Ramseur, NC seeks a part-time Physical Therapist to evaluate, plan and implement PT services for residents under MD orders. You will document findings, formulate treatment plans, and ensure proper coding, billing and regulatory compliance.

The role requires licensure in NC and a DPT/B.S. or equivalent in Physical Therapy, with experience in a long-term care setting preferred. Flexible schedule and participation in facility committees offered.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form, accurately following guidelines.
• Develop a comprehensive treatment plan with long/short term goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatments using modalities and modify as indicated with safety precautions.
• Document daily participation and progress on weekly notes.
• Update treatment plan and discharge summaries as indicated.
• Document caregiver or discharge in-services as required.
• Complete all documentation forms required by the facility.
• Contribute care plan information for the MDS as needed.
• Attend facility/rehab meetings and participate in committees as indicated.
• Act as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and performing screens.
• Complete billing and bill patients ethically for PT services.
• Cover other therapists’ caseload during absences.
• Supervise junior staff or students as needed; co-sign notes as required.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education on patient care to rehab staff, facility staff, hospitals and community.
• Comply with facility policies, patient confidentiality and Resident Rights.
• Report on time and adhere to scheduled hours; project a professional image.
• Adhere to productivity requirements and HR policies regarding meals, sick leave and time off.
• Maintain a positive work atmosphere and professional communication.
• Perform any other duties as assigned by supervisor or regional manager.

Job Responsibilities:

• 1.Provide a comprehensive physical therapy evaluation based on MD’s orders.

• 4.Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• 5.Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• 6.Document patient’s daily participation and progress on weekly progress note accurately and timely
• 7.Document updated treatment plan in timely manner as indicated.
• 8.Document patient’s discharge and in-services on a discharge summary as indicated
• 9.Document any caregiver or discharge in-services on an in-service form as indicated.
• 10.Complete any and all other documentation forms required by Facility.
• 11.Document care plans and information needed for the MDS as deemed necessary by the facility
• 12.Attend facility and rehab meetings as indicated by Supervisor and/or Regional Manager.
• 13.Participate in facility committee meetings as indicated

• 24.Report to work on time, adhere to scheduled hours and project a professional image at all times.
• 25.Adhere to productivity requirements.
• 26.Comply with all Facility HR policies re: lunch, sick and time off notification.
• 27.Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers
• 28.Perform any other duties assigned by the department supervisor, lead therapist or regional manager.
• 29.Adhere to any and all other Facility written and oral policies and procedures.
