Job summary

Rockwell Park Rehabilitation and Healthcare Center in Charlotte, NC is hiring a Part-Time Physical Therapist to evaluate residents, develop treatment plans, and deliver therapies in a comprehensive, compliant manner. The role requires a PT degree, state licensure, and active credentialing with the APTA.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive PT evaluation based on MD orders.
• Document findings on the evaluation form per guidelines.
• Develop a comprehensive treatment plan with goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatment using modalities and adjust as needed.
• Document daily participation and weekly progress notes accurately.
