Job summary

Rockwell Park Rehabilitation and Healthcare Center in Charlotte, NC is seeking a part-time Physical Therapist to join our Long Term Care team. The PT will perform comprehensive evaluations, develop treatment plans, and coordinate care with physicians and an interdisciplinary team.

The role requires graduation in Physical Therapy and a current North Carolina license. Responsibilities include documenting progress, maintaining patient confidentiality, and ensuring high-quality therapy services for

Qualifications

• Graduate with a degree in Physical Therapy (B.S., DPT, MA or MS) recognized by APTA.
• Holds current license/registration in North Carolina or applicable state.

Responsibilities

• Provide comprehensive PT evaluations based on physician orders.
• Develop and document a treatment plan with long- and short-term goals, frequency and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Administer treatments and adjust plans per patient progress and safety guidelines.
• Document patient participation and progress weekly; update plans and discharge summaries as needed.
• Co-sign notes and supervise junior staff or students as required.
