Job summary

Wallace Rehabilitation And Healthcare Center in Wallace, NC is seeking a part-time Physical Therapist to join a dedicated rehab team in a long-term care setting. You will perform comprehensive evaluations, develop and implement treatment plans, and document progress and outcomes to support resident care.

Qualified candidates hold an accredited PT degree, current state license, and are committed to high-quality patient care, teamwork, and compliance with facility policies.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on weekly progress note accurately and timely.
• Document updated treatment plan in a timely manner as indicated.
• Document patient's discharge and in-services on a discharge summary as indicated.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by Supervisor and/or Regional Manager.
• Participate in scheduling of patients as indicated.
• Complete billing and bill patients ethically and accurately for physical therapy services rendered.
