Job summary

• 12–18 months of related experience preferred.

Responsibilities

• Perform a physical therapy evaluation for each referred patient and develop a care plan to restore function, alleviate pain, and prevent disability.
• Implement and monitor the care plan in collaboration with rehab services.
• Follow Baxter Health policies and comply with Healthstream Policy Manager.
• Coordinate with the director of Rehab Services to promote best practices in the Rehab Services Department.
