Job summary

Beyond Faith Home Care is seeking a dedicated Physical Therapist to provide coordinated care to clients across all ages in home health settings. You will plan, implement, and evaluate care plans to restore or maintain patient well-being following medical referrals and therapy assessments.

You will conduct clinical assessments, document plans of care, and communicate effectively with clients, families and healthcare teams while ensuring regulatory compliance and quality care.

Qualifications

• Two (2) years experience as a PT in a clinical care setting.
• Preferred, Home health experience.
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
• Current driver’s license in good standing.
• Reliable transportation and auto liability insurance.
• Current State professional license as a PT in any/all applicable states.
• Current CPR.

Responsibilities

• Promote the company mission, vision and values at all times.
• Perform clinical assessments; identify patient needs and appropriateness of client.
• Document, prepare/revise and complete a Plan of Care (POC), progress notes, and other clinical records.
• Apply infection control concepts and universal precautions in client care activities.
• Plan and provide treatment for neuro-muscular and orthopedic disorders based on medical referral and therapy assessment.
• Instruct client/family on transfer techniques and equipment use.
• Communicate effectively with clients, families, team members and other health professionals.
• Monitor cases for third‑party payer compliance.
• Complete documentation in a timely manner per company policy.
• Demonstrate commitment through in-service participation and continuing education.
