Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN/LVN) to provide direct patient care in patients' homes across Millbrae and surrounding areas.

The role is per diem with visits paying $50–$60 each. You will follow individualized care plans, educate families, document care, and collaborate with the care team to ensure safe, compassionate home hospice services. California license and CPR are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
