Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide per diem home-based care. Rate is $50-$60 per visit.

You will work to help patients heal, stay safe, and thrive in their own environments, following individualized care plans under the supervision of an RN. Responsibilities include direct patient care in accordance with the Nurse Practice Act, educating families, documenting care, and collaborating with the care team.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN/LVN license in California.
• 3+ years nursing experience; home health or med/surg preferred.
• Current CPR certification with AHA.
• Valid driver's license and reliable insured vehicle; able to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
