Job summary

Affinity Hospice Care Inc. is seeking a per diem Licensed Practical/Vocational Nurse in California to provide direct patient care in patients\' homes.

You will implement individualized care plans, educate families, and collaborate with an interdisciplinary team to ensure safe, compassionate home hospice care. The role requires an active CA LPN/LVN license, CPR certification, and 3+ years of nursing experience, with reliable transportation and a patient-first mindset.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
