Job summary

Affinity Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to join our home-based care team in California. You will provide direct patient care in patients' homes, support families, and help patients stay safe and comfortable.

This per diem role offers flexibility with a per-visit rate of $50-$60, potential to convert to full-time based on performance, and access to ongoing skills training and development. A supportive team environment emphasizes compassionate care.

Qualifications

Graduate of an accredited practical or vocational nursing program.
Active LPN or LVN license in California.
3+ years of nursing experience; home health or med/surg background preferred.
Current CPR certification with AHA.
Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

Provide direct patient care in accordance with the state Nurse Practice Act.
Implement and follow individualized care plans established by the RN.
Educate patients and their families on self-care techniques and diagnosis-related information.
Document care accurately and in a timely manner.
Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
Use supplies and equipment safely, adhering to aseptic technique.
Perform other duties as required for patient services.
