Qualifications

• Active Michigan Physical Therapy license required.
• Reliable transportation to conduct home visits.
• Driver's license and auto insurance in your name required.

Responsibilities

• Provides skilled physical therapy in accordance with physician's plan of care.
• Assists physician in assessing patient functional level and revising plan of care, including OASIS assessments.
• Treats patients to relieve pain, restore function and maintain maximum performance.
• Observes, records and reports patient responses to treatment and condition changes.
• Instructs patient, family and health team on phases of physical therapy and device usage.
