Qualifications

• Licensed in Florida or applicable state.
• At least 2 years of home health experience.
• Level 2 Background screening is required.

Responsibilities

• Provides physician-prescribed physical therapy.
• Improves or minimizes residual disabilities.
• Evaluates patient progress and documents care.
• Collaborates with other health professionals in care planning.
• Performs all skilled procedures as order by physician.
• Completes and submits OASIS assessments and progress notes.
• Instructs patients and family in home programs and activities of daily living.
