Palms Home Care in Orlando, FL is expanding and seeking a skilled Home Health Physical Therapist (PT) to join our clinical team. You will deliver patient-centered PT services in home settings, helping clients improve mobility and regain independence after illness, injury, or surgery.

The role requires a current PT license, strong communication, reliability, and the ability to work independently within Florida AHCA guidelines and EMR systems like Kinnser.

Qualifications

• Licensed PT with active Florida license.
• Graduate of an accredited physical therapy program.
• Experience in home health preferred.
• CPR certification maintained.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
