Job summary

Safe Life Home Health Care in Morris, IL is seeking a Physical Therapist to provide home-based therapy in the surrounding area. The role includes supervising Physical Therapy Assistants and implementing the plan of care in line with agency policies.

The PT will perform initial evaluations within 48 hours of referral, revise plans as needed, and coordinate with physicians and the care team. Active IL license and at least one year of home health experience required.

Qualifications

• Must be licensed as a Physical Therapist in Illinois.
• Minimum one year of home health care clinical experience.
• Must own a car with valid insurance.

Responsibilities

• Assist physician in evaluating patients’ function and establishing treatment goals.
• Perform initial evaluations within 48 hours of referral and re-evaluations every 30 days, coordinating plan of care with the physician as needed.
• Supervise PT Assistants and ensure plan of care follow-through.
• Coordinate care with DON and agency staff.
• Instruct patient and family in home therapy programs and equipment use.
