Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in clients’ homes, promoting independence and mobility. You will work autonomously within a supportive care team to develop and implement customized plans, educate patients and families, and facilitate safe transitions of care.

The role emphasizes field-based, hands-on therapy with strong emphasis on outcomes, documentation in EMR, and collaboration with nurses, therapists, and physicians.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately in the EMR.
• Lead and supervise Physical Therapist Assistants (PTAs) per state guidelines.
