Qualifications

• 1. A degree in Physical Therapy from an accredited university/program.
• 2. A current, unrestricted state license as a Physical Therapist.
• 3. Demonstrated proficiency in clinical assessments, documentation, and compliance with policies.
• 4. Strong organizational and communication skills.
• 5. Experience in Home Health Therapy and OASIS documentation preferred.

Responsibilities

• Conduct comprehensive assessments of patients in their homes.
• Create personalized, evidence-based treatment plans.
