Job summary

Lifespan Therapy is seeking a Home Health Occupational Therapy Assistant to deliver patient-centered care in home settings across Southern California. The OTA will implement plans of care created by the supervising Occupational Therapist and focus on ADLs, home safety, and independence for geriatric patients.

You will schedule visits, document progress, and communicate with the supervising OT to ensure coordinated care.

Qualifications

• Valid California Occupational Therapy Assistant license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled COTA Care: implement plans of care by supervising OT.
• Perform Therapeutic Interventions: focus on ADLs, safety, independence.
• Report Patient Progress: document responses and coordinate with OT.
• Manage Your Visits: set schedule and cadence based on care plan.
• Document Efficiently: use EMR system for notes.
