Job summary

OneWell Health Care seeks a dedicated Caregiver/Personal Attendant in San Jose, CA to provide compassionate, hands-on support to individuals with IDD and Autism in home and community settings.

You will assist with daily living, mobility, medication support, personal care, and documentation while following care plans and regulatory requirements. A valid driver’s license, CPR/First Aid, and flexibility for evenings/weekends are required. Competitive benefits offered.

Qualifications

• High school diploma or equivalent preferred.
• CPR and First Aid certification required or obtained within designated timeframe.
• English proficiency required (verbal and written).
• Valid driver’s license and reliable transportation required for transport roles.
• Experience as Personal Attendant, DSP, or caregiver preferred; experience with IDD/Autism is a plus.

Responsibilities

• Assist with activities of daily living, personal care, mobility, and meal preparation.
• Provide medication assistance as authorized and monitor changes in health or behavior.
• Maintain accurate documentation and follow safety plans and organizational policies.
• Transport participants when authorized and participate in required training.
